DESIGN AN AUTOMATED SYSTEM OF STUDENT MEDICAL RECORD KEEPING AND REGISTRATION

INTRODUCTION

Most of the patient and administrative information that flow throughout the health care system is still recorded on paper, according to an earlier report by cnnmoney.com, only 8% of the nation hospitals and clinic currently use the kind of common computerized record keeping system.

In general, medical records may be on physical media such as film(x rays),paper (note ), photographs, often of different sizes shapes, physical storage of document in problematical as not all document type fit in the same size folder or storage spaces

Hand written medical records can be associated with poor legibility, which can contribute to medical errors.

Paper based patient record hinder flow according to once information which has been recorded within a set of bulky paper records, it may not be readily accessible later and effort to compile more complete paper record are likely to make this problem worse. Paper records can only be use in one place at a time.

The data are only as secure as the paper itself and the entire records which are paged within a record can easily be misplace, damage, lost or stolen. Another case is that, student can take another student card to the clinic for treatment in which the student will be successful in doing that because there is no space in the card where student pix can be fixed.In this automated student medical record keeping and registration system, there are some criteria which must be considered.

1.     Problem definition: these include the method by which the problem to be solved becomes clearly understand by a programmer.

2.     Algorithm design: these include the method by which a solution to the problem is developed in some intermediate form of the solution.

3.     Algorithm implementation


INTRODUCTION

                 In this present era of globalization and advanced technology sufficient record keeping cannot be over emphasized. Imagine the scenario when the manual processes and manual modes of instruction get replaced with electronic systems. One of such replacement can be done in the area of student database record system within a clinic.

       Developing student medical record keeping and registration system software would benefit the school clinic management who can have effortless access to the data securely and more easily. The main rational behind the introduction of computer over manual system is for quick, fast, more reliable and efficient in processing by the medical practitioner, this project will keep records of all the student medical report and their medical history from the day of their registration in the clinic till their day of graduation from the school. This project also facilitates the transfer of data to health care establishment or to any other organization or individual such as insurance company or employer. Well kept records usually reflect the level of care given to a patient by the physician. Therefore medical records can be used as an evaluation tool. The more complete the record, the better they will serve the physician and the patient in the event of any action.  Every patient’s medical record must include the following specific information, Patient’s identity which includes the patient’s first name, last name sex, age, department, hostel name, level etc.


STATEMENT OF PROBLEMS

In spite of the important function of medical record, it has come under severe threat that by the manual system of medical record keeping, this system involves taking down student’s data on pieces of papers, which are then put in to files and filed in cabinet.

Another problem is the missing of pieces of information. At times when you visit the school health center and you left your card at home or the cards are missing, you will not be attended to, unless the student case is very critical before they might ask for the student name and starting searching for name among the numerous files which has been piled up.

After so many interviews clinic attendant have said that, they spent more time looking for student information even more than the time they have for the patients.

Another problem of this manual record is space because manual systems that store hard copies of patient records require enormous amount of space in order to hold all the records. This can be costly to a medical facility, because the space has to be paid for both in construction and upkeep. Computerized records, however, can be stored on systems that take up less space than a single filing cabinet. In this current system, file cabinets are used for keeping individual patient card enclosed in a file. This system is so tedious in tracing a record files, slow in processing and retrieval of the patient details.

AIM AND OBJECTIVES

  The aim of this research work is to design an automated system of student medical record keeping and registration. And the system will have the following objectives:

   i) It provide an avenue that will ease the work service of the clinic personal.

 ii) It will also help to eliminate delays and ensure prompt will, reliable and automation of the report.

iii) Another Important aspect of this project is that new student who have been offered admission will also go to the clinic to register with them and not until the students falls sick before he/she will be struggling to collect card for treatment. Once a student is registered, they will be given a number on a small card just like the matriculation number in which everyone will have and whenever a student is sick or going for medical checkup, they will go with the card for reference purposes.

iv) It also provide a basis for the implementation of the computerized system for the sake of efficiency.

v) Another objective is to test and validate the system.
